DTaP/DTwP – DPT VACCINE



The DPT vaccine, recommended by the Indian Academy of Pediatrics (IAP), is crucial for protecting children against Diphtheria, Pertussis (Whooping Cough), and Tetanus. 


Understanding Diphtheria, Pertussis, and Tetanus

Diphtheria: Airborne, presents with fever, throat pain, neck swelling, and can lead to paralysis, heart failure. India reports a high number of cases globally.
Pertussis (Whooping Cough): Airborne, causes prolonged cough, breathing difficulty, and in severe cases, pneumonia, fits, brain damage, or death.
Tetanus: Caused by contaminated wounds or unsafe delivery practices, leading to muscle spasms and “lockjaw.“


Available Vaccines

Combinations: DPT (Triple antigen), DPT/Hib (Quadrivalent), DPT/HBV/Hib (Pentavalent), and DPT/HBV/Hib/IPV (Hexavalent).
For Older Children & Adults: Tdap (reduced Diphtheria and Pertussis, full Tetanus) and Td (reduced Diphtheria, full Tetanus).

Types of Pertussis Vaccines

Whole Cell P (wP): Older, made of killed B. pertussis germs, includes all components of the bacterium.
Acellular P (aP): Purified form, contains components important for protection against Pertussis.



Vaccine Schedule

Infants: 3 doses at 6, 10, 14 weeks.
First Booster: At 16-18 months.
Second Booster: At 4-6 years.
Adolescents: Tdap at 10-12 years.
Adults: Td every 10 years.
Pregnant Women: Tdap (or Td) between 27-36 weeks of each pregnancy.
Post-Injury: Td for tetanus prophylaxis after skin breaching injuries.


Differences between wP and aP Vaccines

wP Vaccines: Longer duration of protection, reduce disease spread, superior long-term protection.
aP Vaccines: Fewer side effects like fever, pain, and swelling.



Side Effects of DPT Vaccines

Common: Fever, redness, pain, swelling at the injection site, drowsiness, refusal of feeds (resolving within 48 hours).
Serious: High fever (>40°C), fits, hyporesponsive episodes (more common with wP).
When to Be Concerned: High fever, fits, less responsiveness, lethargy, or common side effects lasting over 72 hours.


Who Should Receive the Vaccine

All infants and children, as per IAP guidelines.

Who Should Not Receive the DPT Vaccines

– Children with severe allergies from previous doses or known allergies to vaccine components.
– Children with history of multiple fits and altered consciousness post-vaccination should preferably receive the aP vaccines.
